Eligibility:

  • Literally Homeless (shelter, vehicle, street, empty building, etc.)
  • Individuals or Families
  • Mental Health Diagnosis documented by a Licensed Professional

Assistance:
  • Tenants pay 30% of their income towards rent and utilities.

Goal:
  • Sustain permanent housing, Secure or increase income, secure or increase non cash benefits, to gain stability through a consumer driven treatment plan and peer support.

  • Single Adults
  • Mental Health Diagnosis documented by a Licensed Professional

  • Family member has a Mental Health Diagnosis documented a Licensed Professional

HCRP RRH (HOMELESS CRISIS RESPONSE PROGRAM - Rapid Re-Housing)

Eligibility:

  • Rapid Re-Housing: Homeless (shelter, vehicle, street, empty building, etc.)

SOARS Recovery Housing(Seriously Overcoming Addiction thru Recovery and Support

Eligibility:

  • Must be a Resident of Logan or Champaign County
  • Must have an addiction diagnosis
  • Must be connected to a sober support system
  • Must not have any violent criminal background or be classified as a sex offender
  • Must be employed or ability to get employment

  • SOARS provides those who are very serious about Recovery an environment that encourages support.
  • No treatment is done at SOARS, participants choose services like those offered at Consolidated Care.
  • Rent is based on 30% of the participant's income.

HOMELESS ASSISTANCE PROGRAM (H.A.P.)

  • Individuals and Families
  • Mental Health Diagnosis, Alcohol or Drug Addiction
