PERMANENT SUPPORTIVE HOUSING - MADRIVER/PARK

Eligibility: Literally Homeless (shelter, vehicle, street, empty building, etc.) Individuals or Families with a professionally documented Mental Health Diagnosis

Link: Prioritization Process, referral made by local Mental Health Agency provider

Assistance: Tenants pay 30% of their income towards rent and utilities. Remainder is subsidized. Case Management

Goal: Sustain permanent housing, Secure or increase income, secure or increase non cash benefits, to gain stability through a consumer driven treatment plan and peer support.

PERMANENT SUPPORTIVE HOUSING - RA INC. PSH

Eligibility: Literally Homeless (shelter, vehicle, street, empty building, etc.) Single Adults with a professionally documented Mental Health Diagnosis

Assistance: Tenants pay 30% of their income towards rent and utilities. Remainder is subsidized

Eligibility: Literally Homeless (shelter, vehicle, street, empty building, etc.) Families - Adult who has a professionally documented Mental Health Diagnosis

Assistance: Household pays 30% of their income towards rent and utilities. *Must attend treatment to meet required leverage dollars

HCRP (HOMELESS CRISIS RESPONSE PROGRAM)

Eligibility: Income: 30% of area medium income
                  Prevention: eviction notice
                  Rapid Re-Housing: Literally Homeless (shelter, vehicle, street, empty building, etc.)

Eligibility: Individuals and Families. Professionally documented Mental Health Diagnosis; Alcohol or Drug Addiction
