PERMANENT SUPPORTIVE HOUSING 

Logan County: MADRIVER/PARK 

Eligibility: Literally Homeless (shelter, vehicle, street, empty building, etc.)   Adult in Household with verified Disabling Condition

Link: Coordinated Entry: Access Point to Prioritization Process with Chronically Homeless prioritized; Disability Verified by MHDAS (Logan/Champaign) MHRB (Union) or other Mental Health Provider 

Assistance: Tenants pay 30% of their income towards rent and utilities. Remainder is subsidized with other sources. Case Management

Goal: Sustain permanent housing, Secure or increase income, secure or increase non cash benefits, to gain stability through a consumer driven treatment plan and peer support.

HCRP (HOMELESS CRISIS RESPONSE PROGRAM)

Eligibility: Income: 30% of area medium income
                  Prevention: eviction notice
                  Rapid Re-Housing: Literally Homeless (shelter, vehicle, street, empty building, etc.)

HOMELESS ASSISTANCE PROGRAM (H.A.P.)

Eligibility: Individuals and Families. Professionally documented Mental Health Diagnosis; Alcohol or Drug Addiction
